Provisions for becoming a client:
- We will not provide services such as dog walking, pet
food delivery or in home pet care for clients with Pit
Bulls or American/British Staffordshire Terriers at this
time
- Only the animal(s) applicant has at time of this call
will be covered
- All clients with unsprayed/neutered dogs/cats must
agree to have them spayed/neutered within three months
of becoming a client. Exceptions are senior animals, kittens/puppies
< six months and documented show animals
- All companion animals must be kept current with their
yearly vaccinations. If vaccinations are not current clients
will be responsible for attending to the shots within
3 months of becoming a client. To assist with this process
clients will be provided with a voucher or subsidized
according to income
Preparing application:

Fill in the required fields on this form, and click the
submit button to send this form to The Pet Project. When
we receive it, we will put together an application package
and mail it to you.
While filling out the actual application may not rank high
on the pleasures of having a pet, the questions we’ve
asked and the information you will provide, are essential
to creating a quality and innovative pet care service responsive
to you and your companion animal’s particular needs.
Please read through and completely fill out the application
materials, which include the following:
- Cover Letter
- Fact/FAQ sheet
- General Information
- Financial Disclosure
- Pet Personality Profile,
Medical Information & Food Selection Form –
one for each Companion Animal
- Rules & Regulations
regarding the animal food/supplies service (please keep
for reference
you are responsible for compliance)
Please complete and sign the enclosed application, pet
profile/s, the liaison form, and the client agreement for
basic services. If you need additional space for your responses
you may include a separate page. Please be as thorough as
possible in your responses, especially on the Pet Personality
Profile. No detail is too small or inconsequential to include.
The more detailed your responses are, the better we will
be able to assist you and your pet(s) as we increase the
scope of services.
Once you have completed the application and related forms,
please return them in the enclosed envelope. It is important
that you remember to include the following items when returning
the completed application to us:
- Your application
- The Financial Disclosure
- A Pet Personality Profile
for each companion animal to be covered
- A clear & recent
photograph of each pet (For our records, a single picture
of multiple pets is acceptable).
- A copy of most recent
veterinary papers (You can ask your vet for a recent copy,
or if you do not have a current veterinarian, just note
that on your application).
- A Letter of Diagnosis
of your disease, issued within the past six months, signed
and dated by a health care professional
- Xerox of photo ID (Drivers
License, Florida ID Card). We will make a copy if you
bring in the original.

Once your papers are evaluated we will be in touch with
the results of our evaluation. When you receive your ID
at intake, you will be required to provide us with additional
documentation substantiating certain expenses or income
appearing on your Financial Disclosure Form. |
